Below is our recommended immunization schedule. Feel free to download the CDC handouts for more information about each vaccine.

2 months	Pediarix #1 (DTaP [Diphtheria, Tetanus & Acellular Pertussis], IPV [Polio], and Hepatitis B) HIB#1 (Haemophilus Influenzae B) PCV#1 (Pneumococcal vaccine) Rotavirus #1
4 months	Pediarix #2 (DTaP [Diphtheria, Tetanus & Acellular Pertussis], IPV [Polio], and Hepatitis B) HIB #2 (Haemophilus Influenzae B) PCV #2 (Pneumococcal vaccine) Rotavirus #2
6 months	Pediarix #3 (DTaP [Diphtheria, Tetanus & Acellular Pertussis], IPV [Polio], and Hepatitis B) PCV #3 (Pneumococcal vaccine) Rotavirus #3
9 months	PPD (Tuberculosis test) if indicated.
12 months	HIB #3(Haemophilus Influenzae B) PCV #4 (Pneumococcal vaccine) Hepatitis A #1
15 months	MMR #1 (Measles, Mumps & Rubella) Varivax #1 (Chickenpox) DTaP #4 (Diphtheria, Tetanus & Acellular Pertussis)
18 months	DTaP #4 (Diphtheria, Tetanus & Pertussis) if not given at 15 months Hepatitis A #2
2 years	Hepatitis A #2 if not given at 18 months
3 years	PPD (Tuberculosis test) if indicated
4 or 5 years (Well Checks both years)	DTaP #5 (Diphtheria, Tetanus & Pertussis) IPV #4 (Polio) MMR #2 (Measles, Mumps & Rubella) Varivax #2 (Chickenpox)
